1. Birth Certificate

Requirements

• Duly-filled birth certificate request form (download)
• Copy of passport

2. Death Certificate

Requirements

• Request letter from applicant mentioning details of the deceased – should include full name, date of birth, date of death, cause of death, place of burial and any other information that would assist the retrieval of the correct death certificate and
• Documents of identification to support the request – Passport, I.D, Driver’s license, Birth certificate, Marriage or Death certificate.

3. Marriage Certificate

Requirements

• Duly-filled marriage certificate request form (download)
• Copies of passports of the bride and the groom, certified by Notary Public.

To apply for birth, death or marriage certificates, please send your request to aco@fijiemb.ae and secondsecretary@fijiemb.ae with clean scans (PDF) of the relevant required documents.

4. Change of Name

Requirements

• Duly-filled change of name form  (download);

           – should be witnessed and certified by Notary Public

• Copy of valid Passport, Driver’s license, Fiji Revenue & Customs Service/ Fiji National Provident Fund joint card (FRCS/FNPF Card) or Fijian voter card and
• Copy of current birth certificate

For a child under 18 years of age, the following is required in addition to the above.

• Child’s passport-size photo;
• Child’s current birth certificate and
• If separated/divorced or not available at the time of lodgment of the application form, consent of the other parent is required.

For change of name, the required documents are to be sent via courier to the following address:
