- Oath or Affirmation of Allegiance
- Certified Copy of Birth Certificate
- Certified Copies of Parents Passports, Birth Certificates & Marriage Certificate
3. Former Citizen
Requirements
- Duly-filled Citizenship Application Form
- Oath or Affirmation of Allegiance
- Two Colored and Recent Passport-size Photos
- Certified Copy of Birth Certificate
- Certified Copy of Current Passport
- Copy of Current Citizenship Certificate
- Police Report from Country of Current Citizenship
- Police Report from Country of Residence
